Our Adoption Application can be completed in a number of ways:

1. Click on and fill out the Online Application to the left. We're having problems with our application at this time, so you will need to download a PDF or Word file and email it. Thank you!
2. Click on the Email Application to the left, then copy and paste  inton an email, fill it out and send it to: QCGA

3. Click on the Word Application to the left, complete it, and E-mail it back to the above address.

4. Click on either the Word or PDF Application to the left, print it, answer the questions and mail the application to: QCGA, 9525 New Liberty Road, Walcott, IA, 52773.
